Passport Information for the Orchestra & MRD
The County Clerk has set up Special Passport Days for our Group:
Saturday, December 1, 2012 – 9:00am – 12:00pm
Mention you are with HCRHS Band/Orchestra and pay $1.00 for passport photo
& get free notarizing on the school form
Saturday, December 8, 2012 – 9:00am – 12:00pm
Regular $10.00 fee for photo but free notary available

Passport applications can take 2 months to process (longer after the holidays).
If you need passports for our spring trip we strongly suggest you take advantage of these dates.
Please bring Social Security # and Original Birth Certificate
For Students 15 and under: Must bring documents, both parents with photo ID’s or consent form.
Cost is $80.00 – Check made out to US Department of State –
$25.00 – Execution Fee
This passport is valid for 5 years
For Students 16 and over: Must bring documents, one parent with photo ID
Cost is $110.00 – Check made out to US Department of State –
$ 25.00 – Execution Fee
This passport is valid for 10 years
If you fill out the forms ahead of time DO NOT SIGN THEM, please use black ink
If your current passport expires before 9/21/13 you should renew it for this trip.
